Why a link and not a PDF
A PDF gets forwarded, saved in someone else's gallery, and you no longer control it. A link with an expiry can be switched off — and while it lives, whoever opens it sees what is there today, not what was there the day you sent it.
And if the practice prefers paper, it prints straight from there.
